Terminations, hour reductions, and dependents aging off are COBRA qualifying events; notify your administrator within 30 days.
Notify the health carrier the day of (or day after) a termination to stop future premiums.
Enrollments, dependent changes, and comp changes should post before the next benefits or payroll cycle.
| Status Change | Notify By | Notes |
|---|---|---|
| Termination or resignation | Same day or next business day; COBRA qualifying event notice to plan administrator within 30 days | Carrier notification stops future premiums; COBRA clock starts the day coverage is lost. |
| Reduction in hours below benefits threshold | Before the next benefits/payroll cycle; COBRA qualifying event notice within 30 days | Losing eligibility due to hour reduction is a COBRA qualifying event just like termination. |
| New hire reaching eligibility | Before coverage effective date | Enroll with carriers before the waiting period ends so coverage is active on time. |
| Dependent loss of eligibility (e.g., child turns 26) | Before the coverage end date; COBRA qualifying event notice within 30 days | Dependent aging off coverage is a COBRA qualifying event for the dependent. |
| Leave of absence (unpaid) | Before leave begins | Clarify with carriers whether coverage continues and how premiums will be collected. |
| Promotion, transfer, or compensation change | Before next payroll cycle | May affect retirement plan contribution amounts, life insurance coverage tiers, or deduction levels. |
- Vendor contact list: Name, portal URL or phone number, and notification procedure for each benefits vendor: health, dental, vision, life, disability, 401(k) or other retirement plan, FSA/HSA administrator, and COBRA administrator.
- Employee's current elections: Which benefits the employee is enrolled in and at what coverage level, so you know which vendors to notify for each type of change.
- Effective date of the change: Needed by every vendor to process the update correctly.
- Supporting documentation: Termination letter, offer letter (for promotions/transfers), or other internal record confirming the change and effective date.
- Carrier portals: Most group health, dental, vision, and life insurance carriers use an online employer portal for enrollment changes and terminations. This is typically the fastest method and generates a confirmation record.
- COBRA administrator: Notify by whatever method your COBRA administrator specifies; most have an online portal or email form. Keep the confirmation as evidence the 30-day qualifying event clock was met.
- Retirement plan administrator: Follow the plan's procedures for updating contribution rates, loan status, or terminating active participation.
- Internal notification: Also notify payroll, IT (for system access changes), and any other internal teams affected by the status change, but handle external vendor notifications first given the tighter deadlines.
- Carrier confirmationsCarrier portal confirmations or email confirmations of each termination or enrollment change.
- COBRA notice dateRecord of the date you notified your COBRA administrator of each qualifying event.
- Payroll recordsPayroll records showing when deduction changes took effect.
- Internal documentationCopy of the internal status change documentation (termination letter, offer letter, and so on).
- RetentionKeep benefits-related records for at least 6 years (ERISA's general records retention rule for plan records). COBRA-related records should also be kept for at least 6 years.
Common traps
FAQs
Do we have to notify all vendors for every status change?
No, only the vendors administering benefits that are actually affected by the specific change. A promotion with a salary increase may only require updating the 401(k) administrator and life insurance carrier (if coverage is salary-based); a termination requires notifying everyone.
What if we notify late?
For carrier notifications, late notice means you may owe premiums for the gap period and could face claim liability. For COBRA, missing the 30-day window to notify your plan administrator can start a chain of COBRA violations. Review what happened, notify immediately, and document your process going forward.
- DOL: COBRA Continuation Coverage: covers qualifying events, notification windows, and employer obligations.
- IRS Publication 15-B: Employer's Tax Guide to Fringe Benefits: covers benefit eligibility and taxation as employment status changes.
- DOL: ERISA Overview: governs group health and retirement plan records retention requirements.
